About WaivedLab

WaivedLab is a document-preparation service for the CLIA Certificate of Waiver — the federal certificate a med spa, IV therapy clinic, or small practice needs before running waived point-of-care tests. We prepare the CMS-116 and the state-specific paperwork; you review and sign everything; we track the filing until your certificate arrives.

Who operates this site

WaivedLab is operated by Covo Franco Digital LLC, a Wyoming limited liability company, and run by Amaury Covo, a logistics-company executive based in Cartagena, Colombia. We build and operate document-preparation services: businesses where the work is careful paperwork, clear checklists, and follow-through with government agencies — the same discipline logistics runs on.

What we do — and don't do

  • We do: prepare the CMS-116 (or your state's equivalent in NY/WA), assemble the state overlay checklist, and follow the filing until the fee coupon and certificate arrive.
  • You always: review and sign every form. The federal certificate fee (currently $248) and any state fees are paid by you directly to the agencies.
  • We never: give legal, medical, or laboratory-director advice; decide your test menu; or promise approval. That is not modesty — it is the design of the service.

When a filing raises a question that needs real laboratory judgment — a test-menu decision, a waiver-to-moderate upgrade, anything a surveyor would scrutinize — we don't improvise. We route it to independent, credentialed laboratory consultants we partner with, and we tell you that is what we are doing. Administrative preparation is our job; laboratory science is theirs.
