Privacy Notice

This Notice explains how Covo Franco Digital LLC, the operator of WaivedLab, handles information from the website checker, initial inquiry, and any filing service you later choose to purchase.

Before you submit anything

Do not send patient records or protected health information.

The checker and initial inquiry are business-intake tools. Do not enter patient names, dates of birth, test results, medical-record numbers, insurance information, diagnoses, or other patient-identifying information. If documents are needed after an engagement begins, WaivedLab will first give you separate handling instructions.

Purpose of the initial inquiry

It creates a sales lead—not a filing and not an authorization.

We use the inquiry to understand your clinic's proposed workflow, determine whether WaivedLab may be able to help, respond to you, and prepare a confirmed scope and quote. Submitting it does not create an agency application, authorize WaivedLab to submit anything, or require you to buy the service.

1. Information you provide

The inquiry may collect your name, business email, optional phone number, clinic type, testing workflow, timeline, state, planned test selections, and an optional note. If you engage WaivedLab, we may later collect the business, ownership, location, personnel, test-system, signature, authorization, and agency-correspondence information needed for the accepted filing scope.

2. Checker and website information

The checker evaluates your selections in the browser. If you submit the inquiry, your selected workflow, state, and planned tests are included with that inquiry.

The site may also receive basic technical and attribution information such as the page visited, event type, approximate network information available to hosting providers, UTM campaign fields, and a Google click identifier when present. When Google Analytics is enabled, it processes site and event data; contact-field contents are not intentionally sent in WaivedLab analytics events.

When a campaign parameter or Google click identifier is present, WaivedLab stores a sanitized first-touch attribution record in your browser's session storage. It is limited to the current tab session and is included with an inquiry only if you choose to submit the form. It is not used to store contact details or patient information.

3. How we use information

  • • Respond to and qualify the initial inquiry.
  • • Prepare and explain a scope and quote.
  • • Deliver document preparation if you accept the service.
  • • Record review, signature, and separate written authorization checkpoints.
  • • Confirm the manual agency channel and authorized sender.
  • • Transmit a filing when included, separately authorized, and sent by WaivedLab.
  • • Correct errors, provide support, secure the service, and resolve disputes.
  • • Measure which pages and campaigns lead to useful inquiries.

4. Where information is processed

Inquiry information is sent through WaivedLab's server-side route to the operator inbox and, when configured, the operating CRM. Hosting, email-delivery, inbox/CRM, security, and analytics providers may process limited information for WaivedLab under their own service terms.

After engagement, filing information may be shared manually with the applicable agency only for the requested service and only after the required client review, signatures, and written authorization. Information may be shared with an independent professional when you direct or authorize that disclosure. WaivedLab does not sell the contents of your inquiry.

5. Payments

WaivedLab does not collect payment-card information through this website. If you accept a quote, payment instructions are provided separately. A payment provider may then process payment data under its own privacy terms; WaivedLab retains ordinary transaction records such as payer, amount, date, invoice, and payment status.

6. Retention

We retain inquiries only as long as reasonably needed to respond, follow up, measure the service, maintain security, and resolve disputes. If you engage WaivedLab, filing and authorization records may be kept for service delivery, agency follow-up, accounting, legal claims, and applicable recordkeeping obligations. Backup and provider copies may take additional time to expire.

7. Your choices and requests

You may ask to access, correct, or delete information you submitted, or ask WaivedLab to stop nonessential follow-up. We may need to verify the request and may retain information required for a pending service, authorization record, accounting, fraud prevention, legal claim, or other obligation. Applicable law may provide additional rights based on where you live.

8. Security and data minimization

WaivedLab uses server-side delivery and limits initial inquiry fields to business contact and proposed-service information. No online transmission or storage system can be guaranteed completely secure, so please do not send information the form does not request—especially patient information, passwords, payment-card data, or government identification numbers.

9. Children

WaivedLab is a business service and is not directed to children. The website does not knowingly solicit personal information from anyone under 18.

10. Changes to this Notice

We may update this Notice as the service or its providers change. The effective date above identifies the current published version. A material change will be reflected on this page before it governs new information.
